Social Security Disability, Supplemental Security Income and Optional

State Supplement programs provide monthly income for people with

severe disabilities. SSI provides benefits for those who have low income

and limited resources. SSD (Social Security) is based on one’s work

history.

If you are unable to go to a Social Security office to apply for benefits, call to

arrange a telephone interview. If a disabled person cannot manage his or

her own affairs, a family member, friend or legal guardian may complete the

application. You can shorten the time it takes to complete and process your

claim if you have the following when you apply:

􀁺 Social Security number and proof of age

􀁺 Names, addresses and phone numbers of doctors, hospitals,

    clinics and institutions that treated you and approximate dates of

    treatment (include patient numbers, if known)

􀁺 A summary of where you worked in the last 15 years

􀁺 A copy of your W-2 forms (wage & tax statements), or if you’re self employed,

    your Federal tax returns for the past 2 years

􀁺 Dates of any military service

􀁺 The claim number for any other check you receive (or expect to)

    because of your disability

􀁺 The worker’s death certificate and proof of marriage if you are

    applying as a disabled widow or widower

Determines eligibility for Temporary Cash Assistance, Food Stamps and

Medicaid for families with children and aged, blind or disabled adults,

based on state and federal guidelines.

 

Things to bring to interview:

􀁺 Identification of head of household and/or person attending interview

    (picture I.D.)

􀁺 Proof of residency in this county (proof of address such as bill, etc)

􀁺 Social Security Cards for all members of household applied for

􀁺 Alien Status (I.D. Card) of all members who are not U.S. citizens

􀁺 Proof of household size and composition (number in family;

    how many boys, girls, ages)

􀁺 Resources (bank accounts, car registrations, etc.)

􀁺 Any and all income to all household members; loss or lack of

    income must also be proven. Provide at least 8 weeks proof

    of current income from employment

􀁺 Notes from people providing money or paying your bills

􀁺 Medical bills of members 60+ years who receive SSI, or who

    receive SSD, if you wish to claim medical deductions

􀁺 Child/adult dependent care expenses for allowable deductions

􀁺 Shelter costs, if you wish to claim allowable shelter deductions

􀁺 Rent/mortgage payment, home owner’s insurance, electric, gas,

    taxes/assessments, telephone, heating fuel, water, sewage, etc.
