IDENTIFICATION

BIRTH CERTIFICATE

A copy of your birth certificate can be obtained by written request to the

Bureau of Vital Statistics in the county where you were born. There is

usually a fee for this service. To find the address and phone number of the

Bureau of Vital Statistics in your birth county, look in your local library for the

telephone directory of the city in which you were born. You can also phone

the Bureau of Vital Statistics directly. For the phone number, dial 1, area

code of birth county, and 555-1212 and ask for the Bureau of Vital Statistics

number.

Need photo ID, such as passport or driver's license.

IDENTIFICATION CARD

To get an identification card, you can apply at any driver’s license office.

You must be 12+ years, present your Social Security card and have two

items of identification, one of which must list your full name and date of

birth and a second supporting document such as:

􀂋 passport

􀂋 school record stating date of birth

􀂋 baptismal certificate

􀂋 family Bible record

􀂋 Florida vehicle registration certificate

􀂋 court order which includes legal name

􀂋 voter registration card

􀂋 marriage license or certificate

􀂋 Florida or out-of-state driver’s license

􀂋 military ID card

You must show the examiner two forms of identification. You may use an

original or certified copy of any of the items listed under the section for

IDENTIFICATION CARD information. If you are under 18 years and not

married, one parent or a legal guardian must sign your license application.

You will be required to take vision, road signs, road rules and driving tests.

For the driving test, you will need to have a car and proof of insurance. You

may pick up a study booklet at any of the locations listed above.
