MEDICAL - Immunization Requirements
for Children of Florida
Prior to entry, attendance, or transfer to public preschools, schools (kindergarten thru 12th grade), a licensed child care facility or family day care home, each child is required to have:
v Diphtheria Tetanus Pertussis (DPT)
§ Five (5) doses:
§ If 4th dose was given on or after the fourth (4th) birthday, only four (4) doses required.
v Oral Polio Vaccine (TOPV)
§ Four (4) doses:
§ If 3rd dose was given on or after the fourth (4th) birthday only three (3) doses required.
v Measles, Mumps, Rubella (MMR)
§ Two (2) doses of Measles for Pre-K thru 4th grade and 7th grade
§ One (1) dose of Measles for 5th thru 12th grade excluding 7th grade
§ One (1) dose of Mumps
§ One (1) dose of Rubella documented (preferably as 2 MMR) for all grades
Seventh Grade School Entry Requirements:
v Hepatitis B vaccine series
v TD Booster
v Second dose of Measles vaccine
All kindergarten students must have completed the above requirements, with the exception of the Hepatitis B series by the first day of school. The first Hepatitis B shot must be completed to enter school. It is strongly recommend to have and the first and second Hepatitis B shots before starting school.
All immunizations must be documented on a current Florida Certificate of Immunization, Form 680.
VACCINES ARE PROVIDED AT NO COST BY THE BROWARD COUNTY HEALTH DEPARTMENT.
